Lascassas Student Ministry Medical Release Form

Please click the button below to download/print the LBC Medical release form. This form will need to be notarized and forms expire after one year. Please remember to attache a copy of the student's medical insurance to the release form. Thank you!

Financial Assistance Application

We never want fiances to prevent students from attending LSM events. Your financial assistance request will be kept confidential. After applying for assistance you will receive a response from the student pastor. Thank you!
