LSM Event Registration
To register your child for one of our events, just click on the picture of the event below and it will take you to Realm to finish registration. Note that in addition to submitting this form - you will need to fill out all pertinent forms for the specific event(s) AND a current LBC Medical Release Form which needs to be notarized. The LBC medical release form is good for a year. Please be aware when you register your child you are also agreeing to pay a non-refundable deposit for the trip(s) either by cash or check in order to finalize your registration process. You can submit deposits in the youth drop box on the information/resource wall. The reason these are non-refundable is due to registration cost for these events that are often non-refundable to the church. Thank you!
